If the patient's bill will be paid by a third
party accredited with POI, please bring the following documents
and present them to the Credit and Collection window to expedite
processing:
A) If the company of the patient pays
for the bill:
- Letter of Authority
- Company ID
B) If an HMO pays for the patient's bill:
- Referral Slip from the HMO
- HMO membership ID
C) If an insurance company pays for the
patient's bill
- Letter of Authority
- Insurance membership ID
